available to delegates registered for
TCT 2006 only. Room requests for nonregistered
applicants will not be processed. Contact

Visa
Requirements and Letter of Invitation
If you require a visa
to enter the United States, please check
with your local American embassy or consulate
to obtain the necessary documents. Please
allow enough time to process your visa.

The organizers of Transcatheter
Cardiovascular Therapeutics 2006, sponsored
by the Cardiovascular Research Foundation,
believe that international participation
and contributions are critical to the
continued success of this important educational
symposium in interventional vascular medicine.
We also recognize that the application
process for a travel visa has become more
complicated and expensive. We are committed
to assisting our international participants
in this process. The Cardiovascular Research
Foundation is pleased to offer a $100
discount to attendees who must obtain
a visa for travel to the US. This discount
will be applied for registrants from those
countries for which the United States
requires a visa. The list can be found
at www.unitedstatesvisas.gov.
The Cardiovascular Research Foundation
reserves the right to determine individual
eligibility for this offer and to modify,
and/or cancel, this offer at any time.
On request, the Cardiovascular Research
Foundation is pleased to send an invitation
for participation in TCT 2006. Such an
invitation is not a commitment on the
part of the Cardiovascular Research Foundation
to provide any financial support, nor
is it a guarantee of the acceptance of
an abstract, nor a guarantee that the
consulate will issue the visa. While we
are pleased to provide documentation of
your registration to attend TCT, visa
issuance is at the sole discretion of
the consular officer. Letters of invitation
are sent to only those individuals who
are registered to attend TCT 2006 and
whose payments have been verified. To
request an invitation letter, please contact
the TCT registration bureau .
Cancellation
Policy
Cancellations received
in writing by September 15, 2006 will
be refunded less a $100 administrative
fee. Cancellations received in writing
between September 16 and October 6, 2006
will be subject to a 50% penalty. No refunds
will be given after October 6, 2006. Refunds
or credits will not be given for failure
to attend, late arrival, unattended events,
or early departure.
The Cardiovascular Research
Foundation is not responsible for airfare,
hotel, or other costs incurred by participants
in the event of program or registration
cancellation.
Please note, in the
event that the 18th Annual Transcatheter
Cardiovascular Therapeutics symposium,
also known as TCT 2006, is abbreviated
or canceled because of circumstances beyond
the Cardiovascular Research Foundation’s
control, including, but not limited to,
civil disturbance, earthquake, electrical
outage, explosion, fire, freight embargo,
strike or labor unrest, flood, hurricane,
tornado or other acts of God, an act of
war, terrorism, or the act of any government
(de facto or de jure), or any government
agency or official, the Cardiovascular
Research Foundation reserves the right,
at the Cardiovascular Research Foundation’s
sole discretion, to unilaterally terminate
the symposium. In these circumstances,
the registrant expressly agrees to waive
any claim that he, she, or it may have
against the Cardiovascular Research Foundation
for damages or compensation, including,
but not limited to, fees for registration,
housing, airfare, and/or incidental charges
.

Please note: Self-Assessment and Board Review Course

Therapy is not included in TCT Week.

meeting only .

people who choose to register for Scientific

Symposium ONLY, will NOT be admitted to
Sunday workshops and/or scientific sessions

you may not pick up your badge until after
4:30 PM on Wednesday, October 25, 2006.

not pick up your badge until after 4:30
PM on Monday, October 23, 2006. If you

What do you

for exhibit hall ONLY?
If you are registered
for the exhibit hall ONLY, you will be
admitted to the exhibit hall on Level
2, Monday, October 23, Tuesday, October
24, and Wednesday, October 25, 2006 during
official exhibit hall hours. Exhibit hall
only registration does NOT include admittance
to the lower level exhibits (book publishers
and societies) (no exceptions).
